AmizaCare collects information so support requests can be reviewed safely and clearly. We handle personal information with care and reference the Australian Privacy Principles generally.

What information we collect

Website forms and referrals may collect names, contact details, suburb, plan type, NDIS number if provided, requested supports, goals, risks, communication preferences, preferred times and consent details.

Why we collect it

We use this information to respond to enquiries, check service fit, understand participant goals, plan safe support, prepare service agreements and communicate with approved people.

Consent [ 03 ]

Consent and sharing

Referral information should only be submitted where the referrer has consent or authority to share it. Information may be shared with participant-approved people such as nominees, family members, plan managers, support coordinators or workers involved in delivering support.

Disclosure [ 04 ]

When sharing may be required

Information may be shared with authorities where legally required, including where safety, incident, safeguarding or reporting obligations apply.

Direct email communication Send concerns directly to support@amizacare.com.
Identify participant & key contact Specify the participant name, details of your concern and preferred return contact methods.
Manage consent & authority Notify us immediately if approved contact nominees or representation authorities have changed.
Emergencies and immediate danger Do not send urgent safety matters via email. For active danger or immediate medical support, call 000.

Form information and submissions

Transparency on how information entered on our website is processed and secured.

Referral Form
Sends participant and referral details entered into the form to AmizaCare through Formspree.
Contact Form
Sends enquiry details entered into the form to AmizaCare through Formspree.
Third Party
Formspree is used as the website form processor. Formspree receives form submissions so they can be delivered to AmizaCare. See Formspree's privacy policy.
Website Forms
AmizaCare should manage submitted information according to its privacy and consent obligations, including only collecting information needed to respond, check support fit and plan safe next steps.
Sensitive Data
Website forms are useful for enquiries but may not be suitable for urgent or highly sensitive information without considering privacy and consent. Do not use website forms for emergencies or immediate safety concerns.
