Make a referral →AmizaCare accepts self-managed, plan-managed and NDIA-managed participants. The plan type changes how invoices, claims and provider choice usually work.
The participant or nominee manages invoices and claims. Provider choice can be broader, but records and payments still need to be managed carefully.
A plan manager pays invoices from the participant plan and helps track budgets. AmizaCare can send invoices to the plan manager where agreed.
NDIA-managed participants generally use registered providers for relevant supports. AmizaCare can discuss service fit and claiming arrangements.
A service agreement should describe the supports, responsibilities, pricing approach, cancellation arrangements, communication, complaints and how changes will be handled.
