Participant needs

Goals, daily routines, communication preferences, mobility, health needs, behaviour support and risk information.

Step 02

Documentation

Support plans, health plans, medication information, behaviour support plans or other relevant documents where needed.

Step 03

Worker fit

Skills, checks, training, availability, boundaries and whether the worker can safely deliver the requested support.

Step 04

Consent & privacy

Who AmizaCare can speak with, what information can be shared and how the participant wants communication handled.
